Wala pang matibay na batayan upang ilagay sa Alert Level 4 ang anomang bahagi ng bansa kabilang na ang Metro Manila. Gayunman, sinabi ng Malacañang na hindi magdadalawang-isip ang pamahalaan na higpitan pa ito sakaling tumaas pa ang health care utilization rate sa bansa.
